CONDUCT OF EVALUATIONS. 1. The performance of each employee shall be evaluated in writing by the employee’s supervisor on the form included at Appendix B. 2. A probationary employee shall be evaluated no earlier than ten (10) days after employment and re-evaluated no less than five (5) days prior to the completion of the probationary period. 3. Each employee shall be notified at least two (2) days prior to the evaluation.
