Review of File. 1. Each teacher shall have the right upon written request to review the contents of his or her personnel files. The teacher shall be entitled to have a certified representative (from within the system) of his choosing accompany him during such review. If the teacher desires single copies of any of the contents so examined in his or her file, it shall be permitted upon written request and at the expense of the employee.
2. Confidential credentials, letters of reference from universities, individuals, or previous employers are specifically exempted from review unless the Board or any of its agents, supervisors, or administrators rest any decision or discipline in any form on the contents of such confidential credentials or said letters of reference, and under those circumstances the teacher shall have a full opportunity to examine such documents and offer such explanation as he or she deems necessary. Materials placed in the teacher’s personnel file shall be read and signed by the teacher (note the exceptions above). Such a signature merely acknowledges it has been read and does not necessarily indicate agreement with its contents. The teacher shall have the right to answer any materials filed and the answer shall be attached to a file at the time of its inclusion in the personnel file.
Review of File. 1. Review of personnel files shall be in accordance with law. Employees will be notified of requests to review the personnel file and shall be afforded the opportunity to be present when the file is reviewed.
2. The Superintendent or his/her designee will be present during any review of a personnel file.
3. Any person making a request to review a file must sign and date the Review Completion Record to be maintained in the personnel file.
Review of File. An employee shall be allowed the right of review of his or her personnel file and be entitled to the rights and protections of O.R.C. 1347.01, Personal Information Systems provisions.
Review of File. Upon appropriate request by the employee, the employee shall be permitted to examine the personnel file. The employee shall be provided a reasonable amount of time during working hours to review the file. The employee’s request cannot be unreasonably denied or delayed.

Review of File. Any employee will have the right, per existing law, to review the contents of their personnel and payroll file, excluding pre-employment information; and to have a Union representative present during such review. The file review will be conducted at a time mutually agreeable to the parties.
Review of File. A. An employee shall have the right to review the contents of his/her personnel file upon twenty-four (24) hours notice to the Personnel Department. The personnel file shall be forwarded to the employee’s immediate supervisor or his/her designee at the employee's work location within ten (10) working days after receipt of the employee’s request. All items relating to the employee's initial employment with PEF may be kept in a separate confidential file. A representative of USW may be present during the review at the request of the employee.
B. No derogatory material relating to job performance may be placed in the employee’s file unless the employee has seen it, has had the opportunity to sign it, and the opportunity to attach a statement to it. It is understood that such signature merely signifies that he/she has read the material to be filed and does not necessarily indicate agreement with its contents. At the written request of an employee, any and all derogatory material that is at least three (3) years old shall be removed from the employee’s file.
Review of File. Each employee shall have the right, upon request, to review the contents of said employee’s personnel file and to place therein written reactions to any of its contents. Nothing may be permanently removed from the employee’s file without notification of the teacher in writing. Nothing may be added to the employee’s file unless a copy to be signed is sent to the teacher. Such review shall be at a mutually convenient time and in the presence of a designated employee of the Board. The right to review does not extend to college placement or credential files and evaluation or reference material originating prior to the teacher’s employment in the school district.
