Contractor Reporting Requirements. A. The purpose of this project is to reduce isolation, increase connections, and enhance self-confidence for older adults by bridging the digital divide through providing access to digital resources and information. A third-party evaluator will reach out to the Contractor directly to collect available program data information in a one-time effort during 2023. B. CDA will provide Contractor with a form listing iPads that the Contractor receives. Contractor shall update this form monthly and send updates to CDA until all iPads have been distributed. The data requested on this form shall include but need not be limited to: 1) Demographic information of iPad recipients, if provided by the participants. 2) Contact information for iPad recipients. 3) Confirmation if the iPad recipient would like a digital literacy training provider to contact them regarding using the iPad device and accessing other digital online resources more effectively.
