Contractor Reporting Requirements. OJT Trainee Introduction Form – must be submitted to INDOT at time of hire for approval into OJT Program. OJT Trainee Termination/Completion Form – must be submitted to INDOT whenever a trainee is terminated or successfully completes their training program.
