Common use of Credentialing Requirements Clause in Contracts

Credentialing Requirements. Provider shall comply fully with Health Plan credentialing requirements applicable to Provider and health care personnel employed by or contracted with Provider. Provider shall promptly notify Health Plan of any material changes in Provider’s certification, licensure, or other qualification status. To the extent Health Plan delegates credentialing of practitioners to Provider, Health Plan shall review and approve the credentialing process and will audit the credentialing process on an ongoing basis.

Provider Practitioner shall comply fully with Health Plan credentialing requirements applicable to Provider Practitioner and health care personnel employed supervised by or contracted with ProviderPractitioner. To the extent Health Plan delegates credentialing of practitioners to ProviderPractitioner, Health Plan shall review and approve the credentialing process and will audit the credentialing process on an ongoing basis.
