Dementia. 2.1.1 In line with the Wolverhampton Joint Dementia Strategy 2019-24 the BCF Dementia work stream has the remit to implement/deliver the elements of the Dementia strategy. 2.1.2 This includes the five elements of the Dementia Strategy; Preventing Well, Diagnosing Well, Living Well, Supporting Well and Dying Well. 2.1.3 The Workstream which includes representatives from multiple agencies; will also review existing dementia specific day services, education and awareness training and the health and social care pathway. The aim is to promote greater independence and choice for people with dementia, increasing their self-esteem and encouraging people to maintain good social and personal relationships.
