Common use of Eligibility Procedures Clause in Contracts

Eligibility Procedures. Eligible sick leave may be donated to a certificated employee for a catastrophic illness or injury if all of the following requirements are met: a. The employee has voluntarily donated a minimum of one day to the Catastrophic Leave Bank.‌ b. The certificated employee who is, or whose family member is, suffering from a catastrophic illness or injury requests that eligible leave days be donated and provides verification of catastrophic injury or illness from a doctor, as required by the District. c. The employee has exhausted all of his/her paid sick leave.‌ d. In the event two or more eligible certificated employees are entitled to the benefits of this Catastrophic Leave Bank at the same time, the days will be allocated equally. e. The District, in consultation with the Association, shall make the final approval of the request.
