Eligible Population. The eligible population to be served under this Grant Agreement is: 1. Individuals who are 60 years or older; 2. Individuals with intellectual disabilities, developmental disabilities, physical, and/or chronic diseases or conditions; 3. Family caregivers; 4. Veterans; 5. Families with children with disabilities; and 6. Other individuals planning for future LTSS needs.
