Target Population. Grantee shall provide Coordinated Specialty Care for First Episode Psychosis (CSC- FEP) outpatient services for individuals who are between fifteen (15) to thirty (30) years of age, regardless of the age of the individual at onset, and who meet the criteria for a psychotic disorder under the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision (DSM-5-TR). These diagnoses for psychosis are listed in Attachment A-2, Clinical Diagnostic Eligibility Early Onset Program, of the Grant Agreement.

Target Population. Grantee must, in accordance with Section 306((a)(4)(B)(i) and (a)(6)(G)) of the Older Americans Act of 1965 (OAA) and its subsequent amendments, and as addressed in the approved Area Plan, assure HHSC that it will use outreach efforts to identify persons who are eligible for assistance under this Contract, with special emphasis on: (1) older persons residing in rural areas; (2) older persons with greatest economic need, with particular attention to low-income minority and older persons residing in rural areas; (3) older persons who have greatest social need with particular attention to low-income minority persons residing in rural areas; (4) older persons with severe disabilities; (5) older persons with limited English proficiency; (6) older persons with Alzheimer's disease and related disorders with neurological and organic brain dysfunction and the caretakers of such persons; (7) older persons at risk for institutional placement; and (8) older persons who are Native Americans located within the AAA's planning and service area.
