Common use of Extended Health Clause in Contracts

Extended Health. Following ninety (90) calendar days of service with the company, all employees, including their families, are entitled to benefits under the provisions of the insurance company’s Health Care Plan, with an annual deductible of twenty-five (25) dollars per employee for Single coverage and fifty (50) dollars for Family coverage for all covered expenses except prescription drugs. The prescription drugs portion of the Extended Health Plan will be covered by a generic drug plan card (two (2) cards to be issued per family), provided by the insurance company. Each purchase of prescription drugs under this plan will be subject to a five (5) dollar deductible amount to be paid by the employee at the time of each purchase.
