Extended Service Authorization Decision Timeline i. The interdisciplinary team may extend the three (3) calendar day timeframe by up to five (5) additional calendar days for either of the following reasons: a) The participant, legal decision maker, or other requestor authorized under Article V.K.8.b. requests the extension, or b) The extension is in the participant’s interest because the IDT needs additional information from an individual not directly employed by the PO that may change the IDT’s decision to deny a service. The IDT must document the circumstances that led to the extension and how the delay is in the interest of the participant. ii. The IDT must notify the participant either orally or in writing when the IDT extends the timeline. The notice must explain the reason(s) for the delay and must be issued as expeditiously as the participant’s condition requires, but no later than 24 hours after the IDT decides to extend the timeline.

  • Retainage for Unacceptable Corrective Action Plan or Plan Failure If the corrective action plan is unacceptable to the Department or Customer, or implementation of the plan fails to remedy the performance deficiencies, the Department or Customer will retain ten percent (10%) of the total invoice amount. The retainage will be withheld until the Contractor resolves the performance deficiencies. If the performance deficiencies are resolved, the Contractor may invoice the Department or Customer for the retained amount. If the Contractor fails to resolve the performance deficiencies, the retained amount will be forfeited to compensate the Department or Customer for the performance deficiencies.
