The Extended Services clause defines the terms under which additional services beyond the original agreement may be provided. Extended Services. The Inspector shall provide additional extended Services for the Project made necessary by Construction Work damaged by fire or other Acts of God during construction, or prolongation of the initial construction contract time beyond the construction contract time schedule.

Extended Services a. The purpose of extended services is to provide those ongoing support services that are necessary to maintain the individual in the job, to restore stability as necessary, and to place the individual in a new job when a job is lost, or when appropriate, conduct career development. Ongoing meetings with the individual and monthly contact with the employer are required for routine support, to monitor the individual’s work performance and adjustment, and to determine at the earliest time when destabilization appears to be occurring. b. The Extended Support Plan (ESP) is a tool for the CRP to use when providing ongoing support services. The employment specialist will further develop formal records that describe past, present, and future areas of the individual’s needs as well as strategies for supports. The records must be maintained throughout the individual’s employment tenure. These documents provide a record of individual preferences and successful support strategies. These records must be available to VR upon request for review and monitoring. Other state agency funding sources may need access to these records to verify funding allocation for ongoing services that are not funded by VR. c. An ESP outlines the commitment by a party identified by the CRP prior to stabilization to provide extended services for the client. If the CRP is providing extended services, the CRP must keep timely and appropriate ongoing records for review by VR or DHS staff upon request. d. For youth with disabilities (up through age 24), the CRP will ensure that all possible funding sources for extended services have been explored prior to VR approving the ESP and authorizing to the CRP for extended services. e. When appropriate, the VR Counselor will prepare a letter to the Department of Intellectual and Developmental Disabilities Independent Support Coordinator (ISC) or Employment and Community First (ECF) CHOICES Support Coordinator. This letter will verify that the individual has met the VR stabilization criteria to authorize Medicaid Waiver funding for extended services. Contact the VR SE coordinator for a sample letter. f. Extended services may be funded by VR for youth with disabilities until the individual: 1) No longer meets the established age limit; or 2) Has received extended services for a period of 48 months; or 3) Has transitioned to extended services provided with funds other than VR prior to meeting the age or time limits.
Extended Services. 1.6.4.1 If the Owner and Consultant agree that more extensive service than is described in Paragraphs 1.1 through 1.6 shall be provided and included within Basic Services, the Consultant shall provide additional staff, consulting or manpower as required to provide this additional support. These services are set forth in Exhibit F, Extended Services, which describes the duties, responsibilities and limitations of authority. 1.6.4.2 Such sub-consultants or representatives shall be selected, employed and directed by the Consultant, and the Consultant shall be compensated for them as mutually agreed between the Owner and the Consultant as set forth in Exhibit E, Consultant’s Fee Proposal. 1.6.4.3 Through the observations by such project sub-consultants or representatives, the Consultant shall endeavor to provide further protection for the Owner against project delays, budget and cost overruns, and defects and deficiencies in the Work, but the furnishing of these Extended Services shall not modify the rights, responsibilities or obligations of the Consultant as described in Paragraphs 1.1 through 1.6
