Goals of the MTM Program. The goals listed below must remain at the forefront of the program processes implemented by the Contractor on behalf of Hoosier Care Connect members who are identified as high risk due to a combination of chronic disease state and multiple unique prescribed medications. The MTM program implemented must:  Be patient focused;  Improve medication use;  Reduce risk of adverse events; and  Improve adherence.
