Grievance Form Requirements. All written grievances must contain the following information to be considered: 1. Aggrieved employee’s name; 2. Aggrieved employee’s classification; 3. Date grievance was first discussed; 4. Date grievance is being filed in writing; 5. Name of supervisor with whom ▇▇▇▇▇▇▇▇▇ was discussed; 6. Date and time grievance occurred; 7.
