Health Assessment Sample Clauses
A Health Assessment clause requires parties, typically employees or contractors, to undergo evaluations of their physical or mental health as a condition of engagement or continued participation in certain activities. This may involve periodic medical check-ups, submission of health certificates, or compliance with specific health standards relevant to the role, such as fitness for duty in safety-sensitive positions. The core function of this clause is to ensure that individuals are medically fit to perform their responsibilities, thereby reducing risks to safety, productivity, and legal compliance.
Health Assessment. To obtain an assessment of your health or obtain an opinion about when you will be able to return to work the employer may, at its expense, require you to undergo a medical examination by a doctor nominated by the employer after consultation with you. Where practicable, the employee will be offered a choice about which doctor they are assessed by.

Health Assessment. An employee who regularly works at least three hours of his or her daily working hours between the hours of 11.00 pm and 6.00 am, or who is expected to work at least 300 hours during this time within a 12-month period, must be offered a free health assess- ment by his or her employer. This offer must be provided before the person in question begins night work, and then at regular intervals of no more than 2 years. The health assessment is a voluntary offer. The health assessment should consist of a questionnaire, a subsequent examination with feedback containing advice, and recommendations for the employee. The exami- nation must be carried out by a healthcare professional, e.g. a nurse or doctor, and can take place at the enterprise or at the employee’s own doctor. At larger enterprises where several employees have been offered a health assess- ment, the enterprise will receive an anonymised report detailing general tendencies about the lifestyle and health of the employees.
Health Assessment the ANAO will provide annually, at no expense to the staff member, access on a voluntary basis to a health assessment.

CONTRACTOR shall complete a Health Assessment to screen for the medical, mental health, and case management needs of the client, and the eligibility of an individual to receive services. CONTRACTOR shall examine each Client’s strengths, weaknesses, and resource needs to establish an Individual Service Plan (ISP). The ISP records the C lient’s level of psychosocial impairment, substance abuse and physical health problems, support network availability, adequacy of living arrangements, financial status, and employment status including employment potential and training needs.
