Records Review and Referral a. School will distribute Health Transmittal Assessment Forms to parents/guardians at Kindergarten registration and to students enrolling in a North Carolina public school for the first time. They must have a Health Assessment Transmittal Form completed and presented to their school within 30 calendar days of the student’s first day of attendance. b. School staff will notify the School Nurse of any students with chronic health conditions or mental health issues of which the School Nurse may be unaware. c. School Nurses will collaborate with school staff, parents, physicians, and other necessary disciplines to develop Individualized Health Plans (IHPs) for specific health concerns. Appropriate school personnel will receive communications regarding updates or changes to IHPs.

  • Miscellaneous The Vendor acknowledges and agrees that continued participation in TIPS is subject to TIPS sole discretion and that any Vendor may be removed from the participation in the Program at any time with or without cause. Nothing in the Agreement or in any other communication between TIPS and the Vendor may be construed as a guarantee that TIPS or TIPS Members will submit any orders at any time.
