In accordance with HIPAA (Health Insurance Portability and Accountability Act) all information related to Protected Health Information (PHI), including enrollment forms, will be excluded from an employee’s personnel file due to the confidential nature associated with such information. A copy of the Privacy Notice, as it relates to District group health insurance benefit plans, is available from the receptionist at the Education Services Center (ESC).
