Information Release Authorization. Customer authorizes Renaissance Power & Gas, Inc. to obtain and review information regarding Customer’s credit history from credit reporting agencies and the following information from the LDC: consumption history; billing determinants; account number; credit information; public assistance status; participation in utility low income discount programs; existence of medical emergencies, status as to whether Customer has a medical emergency, is human needs, elderly, blind or disabled and data applicable to cold weather periods under 220 CMR 11.05(4)(c), tax status and eligibility for economic development or other incentives. This information may be used by Renaissance Power & Gas, Inc. to determine whether it will commence and/or continue to provide energy supply service to Customer and will not be disclosed to a third party unless required by law. Information Release Authorization. Throughout the Term, you authorize Homefield Energy to obtain information from the DSP that includes, but is not limited to, your account name, account number, billing address, service address, telephone number, standard offer service type, meter readings, and, when charges hereunder are included on your DSP bill, your billing and payment information. You authorize Homefield Energy to release such information to third parties, including affiliates that need to know such information in connection with your Retail Power service. These authorizations will remain in effect as long as this Agreement is in effect.

Customer authorizes Think to obtain and review information regarding Customer’s credit history from credit reporting agencies in order to determine whether the Customer is eligible for promotional items. The information referenced in this paragraph will not be disclosed to a third party unless required by law. Think will not disclose a customer’s Social Security number and/or account number without Customer’s written consent except for Think’ own collections and credit reporting, participation in programs funded by the Universal Service Fund pursuant to Section 4928.52 of the Ohio Revised Code or assigning a customer contract to another supplier certified to do business in Ohio. The Customer’s social security number, account number(s) or any other customer information will not be released without the customer’s express written consent except in accordance with rules 4901:1-28-04 and 4901:1-29.09 of the Administrative Code. Customer’s execution of this Agreement shall constitute authorization for the release of this information to Think. This authorization will remain in effect during the Initial Term and any Renewal Term. Customer may rescind this authorization at any time by providing written notice thereof to Think or by calling Think at ▇-▇▇▇-▇▇▇-▇▇▇▇ Monday through Friday 8:00 a.m. to 8:00 p.m. ET

Customer authorizes RESI to obtain and review information regarding Customer’s credit history from credit reporting agencies and the following information from the LDC: consumption history; billing determinants; account number; credit information; public assistance status; participation in utility low income discount programs; existence of medical emergencies, status as to whether Customer has a medical emergency, is human needs, elderly, blind or disabled and data applicable to cold weather periods under PSL § 32 (3); and information pertaining to PSL § 33, tax status and eligibility for economic development or other incentives. This information may be used by RESI to determine whether it will commence and/or continue to provide energy supply service to Customer and will not be disclosed to a third party unless required by law. Customer’s execution of this Agreement shall constitute authorization for the release of this information to RESI. This authorization will remain in effect until this Agreement is terminated by Customer or RESI. Customer may rescind this authorization at any time by providing written notice thereof to RESI or by calling RESI at ▇-▇▇▇-▇▇▇-▇▇▇▇. RESI reserves the right to cancel this Agreement in the event Customer rescinds the authorization.
Information Release Authorization. Customer hereby authorizes the local natural gas distribution company (the “GDC”) to release to IDT Energy, and authorizes IDT Energy to obtain from the GDC and review, detailed information regarding Customer’s account(s) including, but not limited to: account number; meter number; consumption history; billing determinants; payment history; credit information; public assistance status; and participation in utility low income discount programs. This information may be used by IDT Energy to determine whether it will commence and/or continue to provide energy supply service to Customer. Customer may rescind this authorization at any time by providing written notice thereof to IDT Energy or by calling IDT Energy at ▇-▇▇▇-▇▇▇-▇▇▇▇. IDT Energy reserves the right to cancel this Agreement in the event Customer rescinds the authorization.
Information Release Authorization. By accepting this contract I authorize Constellation to obtain information from the Utility through the term including, but not limited to, account name, account number, billing address, service address, telephone number, standard offer service type, historical and future electricity usage, rate classification, meter readings, characteristics of electricity service and, when charges under this agreement are included on my Utility bill, billing and payment information from the Utility. I authorize Constellation to release that information to third parties who need to use or be aware of such information in connection with my electric generation service, as well as to its affiliates and subcontractors for marketing purposes. These authorizations shall remain in effect as long as this agreement (including any renewal) is in effect. I may rescind these authorizations at any time by either calling Constellation at 1‐855‐465‐1244 or providing written notice to Constellation. Constellation reserves the right to reject my enrollment or terminate the agreement if I rescind these authorizations, if I fail to meet or maintain satisfactory credit standing as determined by Constellation, or if I fail to meet minimum or maximum threshold electricity consumption levels as determined by Constellation. If I fail to remit payment in a timely fashion, Constellation may report the delinquency to a creditreporting agency.

You authorize Harborside Energy to obtain and review the following information from the Utility: customer accountholder name, customer billing address, customer service address, and historic electricity usage. This information may be used by us to determine whether we will commence and/or continue to provide energy supply service to you and will not be disclosed to a third-party unless required by law. The Enrollment Consent or execution of this Agreement shall be deemed to represent your authorization for the release of this information to us. This authorization will remain in effect during the Initial Term and any Renewal Term of this Agreement. You may rescind this authorization at any time by providing written notice thereof to us, or call Harborside Energy at 1-877-940-3835. Harborside Energy reserves the right to cancel this Agreement on 10 calendar days’ notice in the event you rescind such authorization.

You authorize SmartEnergy to obtain and review information regarding your credit history from credit reporting agencies and other information from your EDC, including but not limited to the following: account name, account number, billing address, service address, telephone number, standard offer service type, historical and future electricity usage, rate classification, public assistance status, meter readings, characteristics of electricity service and, when charges under the Agreement are included on your EDC bill, billing and payment information. This information may be used by SmartEnergy to determine whether it will commence and/or continue to provide electricity to you. Such information may be disclosed to a third-party if (a) required by law; (b) such disclosure is to a third party service provider under confidentiality obligations not to disclose such information and to use it solely for the purpose of providing services to us; (c) in connection with your electric generation service; and (d) to our affiliates and subcontractors for marketing purposes. Your acceptance of the Agreement is an authorization for the release of this information to SmartEnergy. This authorization will remain in effect during the term of the Agreement. You may rescind this authorization at any time by providing Notice thereof to SmartEnergy or calling ▇-▇▇▇-▇▇▇-▇▇▇▇ (toll-free) Monday through Friday from 8:00 a.m. to 7:00 p.m. E.T., and Saturday from 9:00 a.m. to 6:00 p.m. E.T. SmartEnergy reserves the right to cancel the Agreement on fifteen (15) calendar days’ Notice in the event you rescind such authorization.

Customer authorizes Polaris to obtain and review information regarding Customer’s credit history from credit reporting agencies and information from Customer’s LDC, which shall include, but not be limited to, the following: consumption history, billing determinants, account numbers, credit information, public assistance status, existence of medical emergencies, status as to whether Customer has a medical emergency, is needy, elderly, blind, or disabled; data applicable to cold weather periods under section 32(3) of the NY Public Service Law; information pertaining to section 33 of the NY Public Service Law; tax status; and eligibility for economic development or other incentives (collectively, “Customer Information”). This information may be used by Polaris to determine whether it will commence and/or continue to provide energy supply service to Customer and will not be disclosed to a third party unless required by law. Customer’s execution of this Agreement shall constitute authorization for the release of this information to Polaris. Polaris reserves the right to refuse to provide service to Customer under this Agreement if it is unable to obtain the necessary Customer Information or it obtains Customer Information that it considers unsatisfactory. This authorization will remain in effect during any initial or renewal term of this Agreement; provided, however, that Customer may rescind this authorization at any time by providing written notice to Polaris or by calling Polaris at ▇-▇▇▇-▇▇▇-▇▇▇▇. Polaris reserves the right to cancel this Agreement in the event Customer rescinds such authorization. The data obtained pursuant to this authorization may be retained by Polaris for a period of four years post-termination of this Agreement consistent with the statute of limitations for contractual disputes and may be used by Polaris in connection with any ongoing business or legal purpose with respect to its obligations under the Agreement, or to offer additional products or services to Customer during the Term, or any Renewal Terms, or at any time in the four year period after the Agreement terminates, or as contained in any derivative work created by Polaris in association with its business as a retail energy provider.
