INTAKE SERVICES. 1. CONTRACTOR shall assign an intake staff (INTAKE OFFICER) for each child or youth who is brought to the CENTER by a COUNTY Emergency Response (ER) Social Worker and/or a law enforcement (LE) Officer. 2. The INTAKE OFFICER shall work with the ER CSW and LE Officer to complete the intake process. The INTAKE OFFICER must ensure that the ER Social Work shall and/or the LE Officer remain with the child or youth in the CENTER until the intake process and transition has been completed. 3. Intake Form, duly approved or provided by the Department of Family and Children’s Services (DFCS). 4. If notified in advance of a removal by the responding Emergency Response (ER) COUNTY Social Worker (CSW), the INTAKE OFFICER shall obtain information and start paperwork in preparation for child or youth’s arrival. 5. When completing the forms, the INTAKE OFFICER must ensure that the following are received and reviewed for completeness: a. Any paperwork/information from the ER CSW and/or LE Officer; b. A copy of the Juvenile Contact Report (JCR), which is required for all non- dependent children, when law enforcement is involved; and c. A copy of the completed Health Assessment Authorization and Consent Form. 6. The INTAKE OFFICER must check to verify that the correct temporary custody time is documented on the JCR. 7. The INTAKE OFFICER shall conduct a brief interview of the child or youth to verify and secure additional family background information. The INTAKE OFFICER shall likewise use this opportunity to review with the child or youth their personal rights. 8. The INTAKE OFFICER shall then document the child’s personal items using the Clothing Inventory Form, duly approved or provided by DFCS. Thereafter, the INTAKE OFFICER shall take a picture of the child and upload the same into the designated software program. 9. Once all the appropriate documents and the intake process have been completed, the INTAKE OFFICER shall introduce and transition the child or youth to the counselling staff (WELCOMING COUNSELOR) and health specialist (CLINICIAN). 10. DFCS and CONTRACTOR shall determine a system that the INTAKE OFFICER shall use to store all the information from the intake process. The same information shall be provided to DFCS for input into the Child Welfare Services/Case Management System (CWS/CMS). 11.   • Online Services Microsoft warrants that each Online Service will perform in accordance with the applicable SLA during Customer’s use. Customer’s remedies for breach of this warranty are described in the SLA.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.
