Less Frequent Evaluation Cycle of Teachers with Five or More Years of District Teaching or Related Service Experience Receiving an “Accomplished” or “Skilled” Rating. 3.5.4.1 All teachers with five or more years of District teaching or related service experience who received a final holistic rating of “accomplished” on the teacher’s most recent evaluation 3.5.4.2 All teachers with five or more years of District teaching or related service experience who received a holistic rating of “skilled” on the teacher’s most recent evaluation conducted under this policy will have the subsequent evaluation in the second year of a two-year cycle, instead of an annual basis, so long as the teacher and evaluator jointly develop a Professional Growth Plan for the teacher, and the evaluator using documented evidence determines the teacher is making progress on that plan. The evaluation shall be completed by the first day of May of the applicable school year, and the teacher shall receive a written report of the results of the evaluation by the tenth of May of that school year. 3.5.4.3 Consistent with the requirement set forth in the Ohio Revised Code, in any year in which a teacher is not formally evaluated, the teacher will have at least one informal observation (walkthrough) and conference with their evaluator. In that case, the evaluation shall be completed by the first day of May of the applicable school year, and the teacher shall receive a written report of the results of the evaluation by May 10 of that school year. 3.5.4.4 Nothing in this Article 3.5.4 will prevent the District from initiating an evaluation cycle.

  • Term Contract Quarterly Sales Reports The Contractor shall submit Quarterly Sales Reports in the manner and format required by the Department within 30 calendar days after the close of each State fiscal quarter (the State’s fiscal quarters close on September 30, December 31, March 31, and June 30). Initiation and submission of the most recent version of the Quarterly Sales Report posted on the DMS website is the responsibility of the Contractor without prompting or notification from the Department. Sales will be reviewed on a quarterly basis. If no sales are recorded in two consecutive quarters, the Contractor may be placed on probationary status, or the Department may terminate the Term Contract. Failure to provide the Quarterly Sales Report, or other reports requested by the Department, will result in the imposition of financial consequences and may result in the Contractor being found in default and the termination of the Term Contract.

  • Annual Evaluation The Partnership will be evaluated on an annual basis through the use of the Strategic Partnership Annual Evaluation Format as specified in Appendix C of OSHA Instruction CSP ▇▇-▇▇-▇▇▇, OSHA Strategic Partnership Program for Worker Safety and Health. The Choate Team will be responsible for gathering required participant data to evaluate and track the overall results and success of the Partnership. This data will be shared with OSHA. OSHA will be responsible for writing and submitting the annual evaluation.
