Limited Scope. The Specialist acknowledges that the Dental Services are limited to those Dental Services usually and reasonably performed by a dentist pursuant to the specific Dental Services requested by the Practice. Such Dental Services are not to include any treatment outside the scope of that which is requested in the applicable Case Statement.
