Mental Health Records a. The County must have policies and procedures for participating provider medical records content and documentation that have been communicated to providers and a process for evaluating its providers’ mental health records based on the County’s policies. These policies must address patient confidentiality, organization and completeness, tracking, and important aspects of documentation such as accuracy, legibility, and safeguards against loss, destruction, or unauthorized use. The County must also have confidentiality policies and procedures that are applicable to administrative functions that are concerned with confidential patient information. Those policies must include information with respect to disclosure of member-identifiable medical record and/or enrollment information, and specifically provide that: 1) The members may review and obtain copies of medical records information that pertains to them. 2) The policies above must be made available to members upon request. b. Member mental health records must be maintained in an organized manner (by the County, and/or by the County’s subcontractors) that permits effective patient care. They must reflect all aspects of patient care and be readily available for patient encounters, for administrative purposes, and for Department review. c. Because counties are considered contractors of the State and are therefore (only for the limited purpose of obtaining mental health records of its members) entitled to obtain mental health records according to Wis. Adm. Code, HFS 104.01(3), the Department will require BadgerCare Plus-certified providers to release relevant records to the County to assist in compliance with this section. Where counties have not specifically addressed photocopying expenses in their provider contracts or other arrangements, the counties are liable for charges for copying records only to the extent that the Department would reimburse on a fee-for-service basis. d. The County must have written confidentiality policies and procedures in regard to confidential patient information. Policies and procedures must be communicated to County staff, members, and providers. Mental Health Records. Upon admission, the State shall immediately request all pertinent mental health records regarding the inmate's prior hospitalization, court- ordered evaluations, medication, and other treatment. DOJ acknowledges that the State's ability to obtain such records depends on the inmate's consent to the release of such records and the cooperation of health care providers at non-DOC facilities.

Mental Health Records. Contractor warrants that Contractor is knowledgeable of Welfare and Institutions Code section 5328 respecting confidentiality of any information regarding beneficiaries (or their families) receiving Contractor’s services. Contractor may obtain such information from application forms, interviews, tests or reports from public agencies, counselors or any other source. Without the beneficiary’s written permission, Contractor shall divulge such information only as necessary for purposes related to the performance or evaluation of services provided pursuant to this Contract, and then only to those persons having responsibilities under this Contract, including those furnishing services under Contractor through subcontracts. Contractor and staff will be responsible for only accessing beneficiary data from the County’s electronic health record for beneficiaries for which they have open episodes of care and for which individual staff have a specific business purpose for accessing. All attempts to access beneficiary data that do not meet those requirements will be considered data breaches and Contractor is responsible for reporting such breaches to County Quality Improvement and HSS Department Compliance unit immediately or within 4 hours of discovery. In the event of a breach or security incident by Contractor or Contractor’s staff, any damages or expenses incurred shall be at Contractor’s sole expense. Provide technical assistances in the form of phone consultations, site visits and meetings to provide clinical guidance and address challenges in the clinical program, implementation and/or performance of the Contract; Provide training and technical assistance on the use of the Netsmart Avatar electronic health record system (only if vendor will be entering services into Avatar); Provide feedback on performance measures objectives and fiscal expenditures in a timely manner to seek a proactive solution; Provide tools and training support for the implementation of the ACT model to fidelity; Make available electronically all policies and procedures referenced herein and inform the Contractor as policies are reviewed and updated so that the Contractor is aware of changes.

  • Patient Records Upon termination of this Agreement, the New PC shall retain all patient dental records maintained by the New PC or the MSO in the name of the New PC. During the term of this Agreement, and thereafter, the New PC or its designee shall have reasonable access during normal business hours to the New PC's and the MSO's records, including, but not limited to, records of collections, expenses and disbursements as kept by the MSO in performing the MSO's obligations under this Agreement, and the New PC may copy any or all such records.

  • Mental Health Services Grantee will receive allocated funding to secure Mental Health Services and Programs for youth under ▇▇▇▇▇▇▇’s supervision. Services may include screening, assessment, diagnoses, evaluation, or treatment of youth with Mental Health Needs. The Department’s provision of State Aid Grant Mental Health Services funds shall not be understood to limit the use of other state and local funds for mental health services. State Aid Grant Mental Health Services funds may be used for all mental health services and programs as defined herein, however these funds may not be used to supplant local funds or for unallowable expenditure. Youth served by State Aid Grant Mental Health Services funds must meet the definition of Target Population for Mental Health Services provided in the Contract.

  • Project Records Borrower shall: Make and keep books, records, and accounts, in such reasonable detail, so as to fully, accurately, and fairly reflect the activities of Borrower. Record the Project’s assets, liabilities, revenues, expenses, receipts and disbursements in separate accounts from any other assets, liabilities, revenues, expenses, receipts and disbursements of Borrower so as to permit the production of a Statement of Financial Position, a Statement of Profit and Loss (Statement of Activities), and a Statement of Cash Flows for Borrower in which the activities of Borrower are separately identifiable from the activities of the Operator, unless Borrower is also Operator. Devise and maintain a system of internal accounting controls sufficient to provide reasonable assurances that: Transactions are executed, and access to assets is permitted, only in accordance with ▇▇▇▇▇▇▇▇’s authorization; Transactions are accurately and timely recorded to permit the preparation of quarterly and annual financial reports in conformity with applicable Program Obligations; Transactions are timely recorded in sufficient detail so as to permit an efficient audit of the Borrower’s books and records in accordance with Generally Accepted Auditing Standards (GAAS), Generally Accepted Government Auditing Standards (GAGAS), and other applicable Program Obligations; and Transactions are timely recorded in sufficient detail so as to maintain accountability of the Borrower’s assets.
