Claim Forms shall be available from the Settlement Administrator and on the Settlement Website. To make a Valid Claim, Settlement Class Members must: (1) complete a Claim Form, providing all of the information and documentation required by the Settlement Agreement and the Claim Form to the Settlement Administrator no later than one hundred and twenty (120) Days after the Preliminary Approval Date, i.e., the Claim Deadline. Only Settlement Class Members who submit Valid Claims by the Claim Deadline shall be entitled to a Cash Award.
