Notification of Enrollment. PROVIDER agrees that it will not receive payment until the provider has entered the certificate of eligibility number into the Provider Portal and the COALITION has in turn approved the enrollment of the child through the provider portal.

PROVIDER agrees that, after the annual cumulative attendance has been certified, the certified annual cumulative attendance may not be disputed for payment purposes.
Notification of Enrollment. You agree that to receive payment, you will enter the certificate number into the Provider Portal for ELCFH approval of child enrollment.
Notification of Enrollment. The Company and the Supplier will provide written notice of enrollment to the Customer entering into a service agreement with the Supplier.
