Notification of Sickness or Other Absence. 13.1 If you are absent from work for any reason and your absence has not previously been authorised by your Manager you, or someone on your behalf, must inform your Manager by 10.00 am on each day of absence. 13.2 Any unauthorised absence must be properly explained and in the case of an absence of uncertain duration you must keep the Company informed on a daily basis until you have provided the Company with a medical certificate. 13.3 If you are absent from work due to sickness or injury which continues for more than seven days (including weekends) you must provide the Company with a medical certificate on or before the eighth day of sickness or injury. Thereafter medical certificates must be provided to the Company to cover any continued absence. 13.4 Immediately following your return to work after any period of absence which has not previously been authorised by your Manager you are required to complete a Self-Certification form stating the date of and the reason for your absence, including details of sickness on non-working days as this information is required by the Company for calculating Statutory Sick Pay entitlement. Self-Certification forms will be retained in the Company’s records. 13.5 Failure to comply with the above procedures may result in disciplinary action and/or the loss of Company sick pay (referred to below) and may also disqualify you from receiving Statutory Sick Pay.

Notification of Sickness or Other Absence. 11.1 If you are absent from work and your absence has not previously been authorised, you or someone on your behalf must inform the Employer of the reason for your absence and its expected duration as soon as possible, but not later than [two] hours before your working day is due to begin. Notification of sickness or other absence by text message is not permitted. 11.2 Where the absence is because of illness or injury:- 11.2.1 If you are absent for five working days or less, you must complete a self- certificate, available from the Employer, and deliver it to the Employer immediately upon your return to work; and 11.2.2 If you are absent for more than five consecutive working days you must provide the Employer with a medical certificate from your doctor covering your period of absence. Further medical certificates must be provided to the Employer to cover any continued absence. 11.3 Where the absence is for some reason other than illness or injury, you must supply such evidence about the absence and its cause as the Employer may reasonably require.

  • Verification of Sick Leave The appointing officer or designee to whom application for sick leave is made may make such independent investigation as to the necessity for sick leave as is deemed proper and may require certification for any period of sick leave, provided that the employee has been previously notified in writing that such certification for absence of less than five working days shall be required. The Human Resources Director may at any time make such independent investigation as may be deemed proper regarding the illness of any person on sick leave.

  • No Right To Employment or Other Status The grant of this option shall not be construed as giving the Participant the right to continued employment or any other relationship with the Company. The Company expressly reserves the right at any time to dismiss or otherwise terminate its relationship with the Participant free from any liability or claim hereunder.

  • No Applicable Registration or Other Similar Rights There are no persons with registration or other similar rights to have any equity or debt securities registered for sale under the Registration Statement or included in the offering contemplated by this Agreement, except for such rights as have been duly waived.

  • Transfer of Sick Leave Any teacher shall be entitled to transfer sick leave credit from 14 other Florida school districts with the restriction that at least one-half (1/2) of the valid 15 accrued leave shall be established in The School District of ▇▇▇ County, Florida.

  • Persons Affected by Alcohol and/or Other Drugs No person is permitted to work on a building site whilst incapable of performing safe work practices. • If the question arises, the OH&S Committee will determine if a person is capable of performing safe work practices and will take necessary action to ensure that safety is maintained. • If safety is compromised, management may take disciplinary action against the person in question, following consultation with the OH&S Committee and the person’s representative. One verbal warning, one written warning, applies.
