Phase 3 – Reporting. 3.1. Compile information from phase 1 and 2 into a report.
Phase 3 – Reporting. ‌ For Phase 3 funding, municipal transit systems are required to provide two reports back to the Ministry with details of the financial impacts that are being offset by Phase 3 funding and outcomes on the use of the funds. The first report must be submitted to the Ministry by October 26, 2021 and consists of actual impacts between April 1 and September 30, 2021 and forecasted impacts between October 1 and December 31, 2021. This report is intended to provide the Ministry with an interim estimate of financial impacts and aligns with the date by which extension for use of Phase 3 funding into 2022 will need to be requested. Municipalities may request an extension to the Phase 3 eligibility period by emailing the program email account (▇▇▇-▇▇▇▇▇_▇▇▇▇▇▇▇_▇▇▇▇▇▇▇@▇▇▇▇▇▇▇.▇▇) by October 26, 2021. The second report must be submitted to the Ministry by January 31, 2022, and will consist of actual impacts between April 1 and December 31, 2021. This reporting will confirm the actual amount of eligible expenditures incurred during the eligibility period. Should your municipality request an extension to the Phase 3 eligibility period, a third report must be submitted by January 31, 2023 to confirm the actual amount of eligible expenditures incurred during the extended eligibility period. Phase 3 interim and final reporting templates, as well as attestation forms will be provided at a later date. To establish a proper baseline for reporting COVID-19 related impacts that uses a pre-COVID-19 context, municipalities can use their pre-COVID 2020 operating budget when calculating COVID-19 related financial impacts between April 1, 2021 and December 31, 2021. This is the same baseline against which Phase 1 impacts were reported. For systems that operate both conventional and specialized services, the cumulative total impact amounts are to be shown in one reporting document.

Related to Phase 3 – Reporting

  • Progress Reporting a) RECIPIENT must satisfactorily demonstrate the timely use of funds by submitting payment requests and progress reports to ECOLOGY. ECOLOGY reserves the right to amend or terminate this Agreement if the RECIPIENT does not document timely use of funds. b) RECIPIENT must submit a progress report with each payment request. Payment requests will not be processed without a progress report. ECOLOGY will define the elements and frequency of progress reports. c) RECIPIENT shall use ECOLOGY’s provided progress report format. d) Quarterly progress reports will cover the periods from January 1 through March 31, April 1 through June 30, July 1 through September 30, and October 1 through December 31. Reports shall be submitted within thirty (30) days after the end of the quarter being reported. e) RECIPIENT must submit within thirty (30) days of the expiration date of the project, unless an extension has been approved by ECOLOGY, all financial, performance, and other reports required by the Agreement and funding program guidelines. RECIPIENT shall use the ECOLOGY provided closeout report format.

  • Additional Reporting Within seven days after the date of this Agreement, the Recipient shall register in ▇▇▇.▇▇▇, and thereafter maintain the currency of the information in ▇▇▇.▇▇▇ until at least October 1, 2022. The Recipient shall review and update such information at least annually after the initial registration, and more frequently if required by changes in the Recipient’s information. The Recipient agrees that this Agreement and information related thereto, including the Maximum Awardable Amount and any executive total compensation reported pursuant to paragraph 38, may be made available to the public through a U.S. Government website, including ▇▇▇.▇▇▇.
