PROPOSAL FORMS. The Intermediary shall forward every Proposal Form for insurance to ▇▇▇▇▇▇▇ as soon as practicable after the intermediary receives the completed Proposal Form. Proposal Forms must be duly completed and signed by the Proposer(s) where applicable.
