PTO Scheduling. PTO of one (1) employee work week or more may be scheduled up to twelve (12) months in advance utilizing the following procedure: 9.5.1.1 Once every six (6) months, in January and in July, a notification for PTO requests for the following twelve (12) month period of time will be posted by the appropriate supervisor of the unit, or department, as the case may be. The posting will indicate the open dates available for PTO scheduling. All Nurses will be afforded one (1) month from the posting date for the purpose of submitting a PTO request for consideration. 9.5.1.2 Nurses desiring PTO will apply by submitting a written PTO request. The nurse may use PTO up to eighty (80) hours per pay period regardless of their FTE. 9.5.1.3 Within two (2) weeks after closing, the supervisor will notify the applicants for both approved and denied requests for PTO. Nurses submitting multiple requests for PTO must prioritize their requests to facilitate identification of which request will be subject to the seniority provisions below. 9.5.1.4 In the case where all of a Nurse’s requests have been denied due to conflicting requests, alternative dates may be selected by the Nurse, subject to supervisor approval or denial within two (2) weeks. Where more than the allowable number of employees have requested the same PTO, seniority shall govern. During this bidding period each Collection team shall be considered separately for seniority requests. Unsuccessful PTO bidders shall select another PTO period if they so desire before other requests of other employees are considered.
B. After December 31st, PTO requests will be on a first come, first served basis.

F. Requests for PTO. Nurses submitting multiple requests week(s) of PTO inclusive or adjacent to recognized holidays for PTO must prioritize their requests to facilitate identification of which request full-time employees will be subject to the seniority provisions below.
9.5.1.4 In the case where all of a Nurse’s requests have been denied due to conflicting requests, alternative dates may be selected by the Nurse, subject to supervisor approval or denial within treated as separate two-year rotations.
