Purchase or Establishment of New Unit or Location. In the event that, after the Effective Date, ▇▇. ▇▇▇▇▇▇▇▇ purchases or establishes a new business unit or location related to the furnishing of items or services that may be reimbursed by Federal health care programs, ▇▇. ▇▇▇▇▇▇▇▇ shall notify OIG at least 30 days prior to such purchase or the operation of the new business unit or location. This notification shall include the address of the new business unit or location, phone number, fax number, Medicare and state Medicaid program provider identification number and/or supplier number, and the name and address of each Medicare and state Medicaid program contractor to which ▇▇. ▇▇▇▇▇▇▇▇ currently submits claims. Each new business unit or location and all Covered Persons at each new business unit or location shall be subject to the applicable requirements of this IA.
