Purpose of Position. The primary role of the Home Care Packages (HCP) Manager is to implement and manage the Home Care Packages Program following the principles of Consumer-Directed Care and according to the Commonwealth Department of Health program guidelines. The role provides co-ordination of care packages that assist clients to remain living independently in their own homes with appropriate support services. This role requires strong leadership and workforce management skills, a well-developed assessment, care planning and case management skills, and a thorough understanding of contractual and statutory requirements relevant to the role.
