REFERRAL AND INTAKE PROCESS. Additional Services and Access Provisions are listed within the Compliance with DMC and SUBG Requirements Exhibit, Section 2, attached hereto. CONTRACTOR shall follow the referral and intake process as outlined herein: 5.1. ▇▇▇▇▇▇▇▇▇▇ agrees to accept new COUNTY case referrals as having been appropriately screened to the ASAM Level of Care (LOC). 5.2. COUNTY will assess some new/open COUNTY clients with the biopsychosocial assessment instrument and with other SUD screening tools. Probation will also screen for risk assessment for Drug Court and other Probation clients they determine. For COUNTY referred clients receiving Care Coordination services, COUNTY staff may contact the CONTRACTOR for relevant information on client such as treatment completion, attendance, activities that may jeopardize successful treatment completion, etc. CONTRACTOR shall provide information to COUNTY when requested (assuming release of information is signed by client). DRAFT 5.3. CONTRACTOR shall begin client at the level recommended by COUNTY. If CONTRACTOR wishes to change the treatment level, a new ASAM screening shall be completed and consultation with COUNTY case manager and/or SUD supervisor shall be sought before changes are made. CONTRACTOR must request a new authorization for those services that require a COUNTY issued authorization and provide an updated ASAM LOC recommendation with clinical rationale for the change of level request.
