RELEASE FORMS. a. Photo/Video Release Forms. Contractor shall be responsible for distributing permission forms to the parents/guardians of all participants as it relates to photo/video/social media releases. The permission forms must contain language permitting MPS to rely on the same. In the event a participant’s parent/guardian does not grant consent, the participant must still be permitted to take part in all program activities, and it shall be Contractor’s sole responsibility to ensure the privacy of the participant is protected at all times. MPS reserves the right to inspect and make copies of these records at any time. b. Evaluation and Other Data Release Forms.
