Common use of Remote Call Forwarding Clause in Contracts

Remote Call Forwarding. 4.2.11.1 As an option, BellSouth shall make available to Deland Actel an unbundled port with Remote Call Forwarding capability (URCF service). URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

Remote Call Forwarding. 4.2.11.1 4.2.10.1 As an option, BellSouth shall make available to Deland Actel CCI an unbundled port with Remote Call Forwarding capability (“URCF service”). URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel ▇▇▇▇▇▇▇▇.▇▇▇ will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel ▇▇▇▇▇▇▇▇.▇▇▇ the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

Remote Call Forwarding. 4.2.11.1 As an option, BellSouth shall make available to Deland Actel ONS an unbundled port with Remote Call Forwarding capability (URCF service). URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel ONS will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel ONS the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

Remote Call Forwarding. 4.2.11.1 As an option, BellSouth shall make available to Deland Actel Tel West an unbundled port with Remote Call Forwarding capability (URCF service). URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel Tel West will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Tel West the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

Remote Call Forwarding. 4.2.11.1 7.1.10.1 As an option, BellSouth shall make available to Deland Actel Xspedius an unbundled port with Remote Call Forwarding capability (URCF service). When ordering URCF service, Deland Actel Xspedius will ensure that the following conditions are satisfied: 4.2.11.1.1 7.1.10.1.1 That the End User end user of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User end user is different from the URCF service End Userend user); 4.2.11.1.2 7.1.10.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 7.1.10.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 7.1.10.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 7.1.10.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Xspedius the rates set forth in Exhibit A B for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-forward- to number (service). 3. All of the other provisions of the Agreement, dated December 30, 2002, shall remain in full force and effect. 4. Either or both of the Parties is authorized to submit this Amendment to the respective state regulatory authorities for approval subject to Section 252(e) of the Federal Telecommunications Act of 1996. 5. This Amendment is made effective upon the date that it is signed by both Parties.

Remote Call Forwarding. 4.2.11.1 4.2.10.1 As an option, BellSouth shall make available to Deland Actel FLATEL an unbundled port with Remote Call Forwarding capability (“URCF service”). URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel FLATEL will ensure that the following conditions are satisfied: 4.2.11.1.1 4.2.10.1.1 That the End User end user of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User end user is different from the URCF service End Userend user); 4.2.11.1.2 4.2.10.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 4.2.10.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 4.2.10.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 4.2.10.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel FLATEL the rates set forth in Exhibit A B for unbundled local switching, tandem switching, and common transport, including all associated usage usage, incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-forward- to number (service).

Remote Call Forwarding. 4.2.11.1 As an option, BellSouth shall make available to Deland Actel CCI an unbundled port with Remote Call Forwarding capability (URCF service). URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel CCI will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel CCI the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

When ordering URCF service, Deland Actel CityNet will ensure that the following conditions are satisfied: 4.2.11.1.1 4.2.10.1.1 That the End User end user of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User end user is different from the URCF service End Userend user); 4.2.11.1.2 4.2.10.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 4.2.10.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 4.2.10.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 4.2.10.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel CityNet the rates set forth in Exhibit A B for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-forward- to number (service).

When ordering URCF service, Deland Actel Ruddata will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Ruddata the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel Pilgrim Telephone will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Pilgrim Telephone the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

When ordering URCF service, Deland Actel CGI will ensure that the following conditions are satisfied: 4.2.11.1.1 4.2.10.1.1 That the End User end user of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User end user is different from the URCF service End Userend user); 4.2.11.1.2 4.2.10.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 4.2.10.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 4.2.10.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 4.2.10.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel CGI the rates set forth in Exhibit A B for unbundled local switching, tandem switching, and common transport, including all associated usage usage, incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-forward- to number (service).

When ordering URCF service, Deland Actel Delta Phones will ensure that the following conditions are satisfied: 4.2.11.1.1 4.2.10.1.1 That the End User end user of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User end user is different from the URCF service End Userend user); 4.2.11.1.2 4.2.10.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 4.2.10.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 4.2.10.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 4.2.10.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Delta Phones the rates set forth in Exhibit A B for unbundled local switching, tandem switching, and common transport, including all associated usage usage, incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-forward- to number (service).

When ordering URCF service, Deland Actel Conextel will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Conextel the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel Metro Teleconnect will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Metro Teleconnect the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

When ordering URCF service, Deland Actel Think 12 will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Think 12 the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

When ordering URCF service, Deland Actel SPARDI will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel SPARDI the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

When ordering URCF service, Deland Actel BroadRiver will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel BroadRiver the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. Remote Call Forwarding. 4.2.11.1 As an option, BellSouth shall make available to Deland Actel AirCover an unbundled port with Remote Call Forwarding capability (URCF service). URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel Unity Acquisition will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Unity Acquisition the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).

Remote Call Forwarding. 4.2.11.1 As an option, BellSouth shall make available to Deland Actel Clearwave Communications an unbundled port with Remote Call Forwarding capability (URCF service). URCF service combines the functionality of unbundled local switching, tandem switching and common transport to forward calls from the URCF service telephone number (the number dialed by the calling party) to another telephone number selected by the URCF service subscriber. When ordering URCF service, Deland Actel Budget Phone will ensure that the following conditions are satisfied: 4.2.11.1.1 That the End User of the forward-to number (service) agrees to receive calls forwarded using the URCF service (if such End User is different from the URCF service End User); 4.2.11.1.2 That the forward-to number (service) is equipped with sufficient capacity to receive the volume of calls that will be generated from the URCF service; 4.2.11.1.3 That the URCF service will not be utilized to forward calls to another URCF or similar service; and 4.2.11.1.4 That the forward-to number (service) is not a public safety number (e.g. 911, fire or police number). 4.2.11.2 In addition to the charge for the URCF service port, BellSouth shall charge Deland Actel Budget Phone the rates set forth in Exhibit A for unbundled local switching, tandem switching, and common transport, including all associated usage incurred for calls from the URCF service telephone number (the number dialed by the calling party) to the forward-to number (service).
