Common use of Report Forms Clause in Contracts

Report Forms. Provide any report forms and reporting formats required by MDE/OGS at the effective date of this Agreement, and provide to the Contractor any new report forms and reporting formats proposed for issuance thereafter at least thirty (30) days prior to their required usage in order to afford the Contractor an opportunity to review and offer comment.
