Reporting Timeframes. (A) The Contractor shall electronically submit the Department’s MCE Disclosure Form: (1) upon the Contractor submitting a proposal in accordance with the State’s procurement process; (2) upon the Contractor executing the Contract with the Department; (3) upon renewal or extension of the Contract; (4) within 35 calendar days after any change in Persons with Ownership or Control Interest; and (5) within 35 calendar days after any change in Managing Employees. (B) The Department shall review the ownership and control disclosure submitted by the Contractor and any of its Subcontractors as required in 42 CFR § 438.608(c).
