Required Physicals Clause Samples

The Required Physicals clause mandates that certain individuals, such as employees or participants, must undergo physical examinations as a condition of employment, participation, or continued involvement in an activity. Required Physicals. The District will pay the cost of any physical examinations required by the District not to exceed a total cost of $120.00. The District shall have the right to select the place and type of physical provided the District pays the total cost of the physical. Department of Transportation (DOT) physicals will only be paid for by the District for those who are primarily classified as a “School Bus Driver or Van Driver”.
Required Physicals. ▇▇▇▇▇ agrees to pay the cost of physical examinations associated with an employee maintaining their driving credentials. ▇▇▇▇▇’s payment is based upon utilizing ▇▇▇▇▇’s authorized physician.
Required Physicals. A payment of $160.00 will be made toward the cost of a physical examination when required. (Bus drivers are required to get a Department of Transportation physical every two years.) If a driver is required to get a physical every year, a maximum of $80.00 per year will be reimbursed by the district.

  • Annual Physical The Executive may, if the Executive so elects, within the twelve (12) months following the Date of Termination, receive an annual physical at the Company’s expense consistent with, and subject to the requirements of, the physical provided under the Company’s annual physical program as in effect immediately prior to the Date of Termination.

(4) Cooperate with the Trust’s independent accountants and take all reasonable action in the performance of its obligations under this Agreement to ensure that the necessary information is made available to such accountants for the expression of their opinion on the Fund’s financial statements without any qualification as to the scope of their examination.

  • Physical Exams The Employer agrees to provide without cost to employees, physical examinations and/or other appropriate tests when such tests are deemed necessary by management to determine whether the health of employees is being or has been adversely affected by exposure to potentially harmful physical agents, toxic materials, or infectious agents, or by attacks and assaults. The Employer agrees to provide to each affected employee who requests it a complete and accurate written report of any such medical examination or other appropriate tests related to occupational exposure. Additionally, written results of an industrial hygiene measurements or investigations related to an employee's occupational exposure will also be provided, upon request, to the employee or the employee's authorized representative. The Union and/or members of the applicable Health and Safety Committee will be provided copies of summary reports, but such reports will not contain personally identifying information.
