The DORS Staff Specialist for Community Rehabilitation Programs and YOUR SUPPORT SERVICES Executive Director shall act as the administrators of this Cooperative Agreement. These individuals will work collaboratively to facilitate an effective working relationship between the agencies. 3. To provide an effective working relationship between DORS and YOUR SUPPORT SERVICES, there will be exchanges of information and access to records of persons served including information about eligibility, service provision and outcomes. DORS counselors shall receive training in guest access to the public mental health program’s electronic case management system (Beacon Health Options). Confidentiality of all case material shall be preserved and all exchange of information and access to records of persons served will be in accordance with federal and State law and regulation. 4. A review of the Cooperative Agreement will be conducted by the Staff Specialist for Community Rehabilitation Programs, DORS field staff and a representative of YOUR SUPPORT SERVICES prior to expiration. If changes are needed during the period of the Agreement, it may be amended. 5. DORS shall at least annually determine if YOUR SUPPORT SERVICES has provided the approved services to DORS consumers in assessing whether YOUR SUPPORT SERVICES continues to be in an active status.

The DORS Staff Specialist for Community Rehabilitation Programs and SHARED SUPPORT MARYLAND’s Executive Director shall act as the administrators of this Cooperative Agreement. These individuals will work collaboratively to facilitate an effective working relationship between the agencies. To provide an effective working relationship between DORS and SHARED SUPPORT MARYLAND, there will be exchanges of information and access to records of persons served including information about eligibility, service provision and outcomes.
