Responsibility for Verification. The CSG SO shall be responsible for submitting to Black Hills Energy, complete and fully executed forms for each CSG Low-Income Subscriber in the CSG PV System, namely: (1) a “Low Income Verification” form; (2) a Low-Income Subscriber Agency Agreement; and (3) a “Consent to Disclose Utility Data” form.
