SERVICE PROVIDER LOCATIONS. The CRS program is state-wide, but specific provider service locations must be individually approved by HHSC. Contractor agrees that the individual’s services provided under the resulting Contract shall be provided at the approved locations specified on the service authorization form issued by HHSC. If Contractor wishes to add or remove an approved location, Contractor may request this modification via written request. HHSC, in its sole discretion, may approve or reject changes to the approved location.
SERVICE PROVIDER LOCATIONS. The Service Provider shall provide Services solely from the locations set forth in Schedule 2, Exhibit G (Service Provider Delivery Locations). Notwithstanding the foregoing, ▇▇▇▇▇ agrees and acknowledges that Service Provider has implemented a Flexi-Office-Remote working model/policy for its employees (“FOR Model”). Under the FOR Model, Service Provider personnel are permitted to work in flexi (hybrid) mode or fully in office mode or fully remote mode. Accordingly, the Parties may mutually discuss and agree on the percentage of Service Provider personnel working in FOR Model for the provision of the Services. The foregoing will not affect Service Provider personnel’s obligation to comply with the terms and conditions under this Agreement, including performance obligations, and adherence to applicable Aspen policies and procedures.
