Common use of SERVICE USER HEALTH RECORDS Clause in Contracts

SERVICE USER HEALTH RECORDS. 1. The Provider must create, maintain, store and retain Service User health records for all Service Users. The Provider must securely store and retain Service User health records for the periods of time required by Law and securely destroy them thereafter in accordance with any applicable Guidance. B13.2. The Provider must: a) use Service User health records solely for the execution of the Provider’s obligations under this Contract; b) give each Service User full and accurate information regarding his/her treatment and Services received and must evidence that in writing in the relevant Service User health record. B13.3. The Provider must at all times during the term of this Contract have a Caldicott Guardian and shall notify the Authority of their identity and contact details prior to the Service Commencement Date. If the Provider replaces its Caldicott Guardian at any time during the term of this Contract, it shall promptly notify the Authority of the identity and contact details of such replacements. B13.4. Subject to Guidance and where appropriate, the Service User health records should include the Service User’s verified NHS number.
