Assignment to, and Authorization to Deduct and Pay Union Initiation Fee to, Lodge No. 912 (UO), Affiliated with District 34, International Association of Machinists and Aerospace Workers I further hereby assign, from my earnings now or hereafter payable to me from the General Electric Company, to Lodge No. 912 (UO), affiliated with District 34, International Association of Machinists and Aerospace Workers, the applicable fee required for membership and I hereby authorize and direct you to deduct such sum from my earnings and pay the same for my account to the Lodge.
