Simple Extractions. This plan covers the simple extraction of an erupted tooth that does not require a surgical procedure. This plan covers therapeutic pulpotomy for primary teeth. This plan covers biopsies and examinations of hard or soft oral tissue. This plan covers root canal therapy for all permanent teeth, excluding final restoration.

Simple Extractions. This plan covers the simple extraction of an erupted tooth that does not require a surgical procedure. This plan covers therapeutic pulpotomy for primary teeth. This plan covers biopsies and examinations of hard or soft oral tissue. This plan covers root canal therapy for all permanent teeth, excluding final restoration. This plan covers periodontal maintenance, following documented periodontal surgery, two (2) times per plan year if at least three (3) months have passed since the completion of active periodontal surgery. This plan covers periodontal scaling and root planing once (1) per thirty-six (36) month period, per quadrant. Enhanced benefits for these services may be available for members diagnosed with diabetes and/or coronary artery disease.   • Improvement Plans A professional improvement plan is a clearly articulated assistance program for a teacher whose student growth measure dimension of the evaluation is below the expected level of student growth. For the purposes of this agreement, improvement plans shall be based on the individual student growth measure level, and not for overall subjects or classes taught.

  • As-Built Plans Within thirty (30) days after completion of Tenant’s Changes requiring the submission of plans to Landlord, Tenant shall furnish to Landlord a complete set of “as-built” plans and specifications.

  • Complete Work without Extra Cost Except to the extent otherwise specifically stated in this contract, the Contractor shall obtain and provide, without additional cost to the City, all labor, materials, equipment, transportation, facilities, services, permits, and licenses necessary to perform the Work.

  • Contractor Changes The Contractor shall notify DAS in writing no later than ten (10) Days from the effective date of any change in: a. its certificate of incorporation or other organizational document; b. more than a controlling interest in the ownership of the Contractor; or c. the individual(s) in charge of the Performance. This change shall not relieve the Contractor of any responsibility for the accuracy and completeness of the Performance. DAS, after receiving written notice by the Contractor of any such change, may require such agreements, releases and other instruments evidencing, to DAS’s satisfaction, that any individuals retiring or otherwise separating from the Contractor have been compensated in full or that provision has been made for compensation in full, for all work performed under terms of the Contract. The Contractor shall deliver such documents to DAS in accordance with the terms of DAS’s written request. DAS may also require, and the Contractor shall deliver, a financial statement showing that solvency of the Contractor is maintained. The death of any Contractor Party, as applicable, shall not release the Contractor from the obligation to Perform under the Contract; the surviving Contractor Parties, as appropriate, must continue to Perform under the Contract until Performance is fully completed.

  • Data shared with Subcontractors If DSHS Data provided under this Contract is to be shared with a subcontractor, the Contract with the subcontractor must include all of the data security provisions within this Contract and within any amendments, attachments, or exhibits within this Contract. If the Contractor cannot protect the Data as articulated within this Contract, then the contract with the sub- Contractor must be submitted to the DSHS Contact specified for this contract for review and approval.
