STANDARDS OF PROFESSIONAL PRACTICE. 10.1 Health care services shall be made available to Members without discrimination on the basis of type of health benefits plan, source of payment, sex, age, race, color, religion, national origin, health status or disability. Physician shall provide health care services to Members in the same manner as provided to their other patients and in accordance with prevailing practices and standards of the profession.

In the event that any disagreement arises concerning the standards of professional practice maintained by Provider or the quality of Services rendered by Provider, such disagreement shall be subject to a good faith meeting between the parties to resolve such disagreement.

STANDARDS OF PROFESSIONAL PRACTICE. 11.1 PPM agrees to require PPM Physicians to provide Members with medical services which are within the normal scope of PPM Physicians' medical practices. These services shall be made available to Members without discrimination on the basis of health care benefit plan, source of payment, sex, age, race, color, religion, national origin, health status or other handicap, and in the same manner as provided to PPM Physicians' other patients. PPM agrees to require that PPM Physicians provide medical services to Members in accordance with the prevailing practices and standards of the profession. 11.2 PPM understands and agrees and shall require PPM Physicians to agree that HUMANA may deny payment(s) for medical services rendered to Members PHYSICIAN PRACTICE MANAGEMENT PARTICIPATION AGREEMENT
STANDARDS OF PROFESSIONAL PRACTICE. A. In the event that any disagreement arises concerning the standards of professional practice maintained by Provider or the quality of Services rendered by Provider, such disagreement shall be first referred to the Executive Committee of Hospital's medical staff for its review and recommendation. Should Provider find a recommendation of the Executive Committee to be unacceptable, Provider may, at its option, but not more than thirty (30) days after receipt of such recommendation, seek arbitration as per Section 13 of this Agreement, or terminate this Agreement. B. In the event that any disagreement arises concerning the standards of professional practice maintained by Hospital or the quality of services rendered by Hospital which affects Provider or its reputation, such disagreement shall be addressed according to the specific policies and procedures currently in place and being utilized within Hospital, and if not resolved satisfactorily to Provider within thirty (30) days of the complaint, the disagreement shall be subject to arbitration as per Section 13 of this Agreement.
STANDARDS OF PROFESSIONAL PRACTICE. The SMS™ shall: (a) Obtain all appropriate licensing as required by state and local governments. (b) Obtain and maintain both general liability and professional liability insurance coverage, and hold the Institute of Structural Medicine harmless from any claims, liability or judgment arising out of the SMS™’s activities as a practitioner of Structural Medicine.™
STANDARDS OF PROFESSIONAL PRACTICE. 12.1 IDS shall require IDS Hospitals, IDS Physicians, and IDS Providers to provide Members with medical services which are medically necessary and are within the normal scope of Hospital, Physician and/or Provider Services, and within the normal scope of IDS Hospitals’, IDS Physicians’ or IDS Providers’ health care practices, respectively, and as further described in this Agreement and the Attachments appended hereto. These services shall be made available to Members without discrimination on the basis of health care benefit plan, source of payment, sex, age, race, color, religion, origin, health status or other handicap, and in the same manner as provided to IDS Hospitals, IDS Physicians, and IDS Providers’ other patients. IDS shall require IDS Hospitals, IDS Physicians, and IDS Providers to provide health care services to Members in accordance with the applicable prevailing practices and standards of the medical profession. 12.2 IDS understands and agrees that and shall require IDS Hospitals, IDS Physicians and IDS Providers to agree that HUMANA may deny payment for medical services rendered to Members which are determined, in the opinion of HUMANA, not to be medically necessary. Medically Necessary (or “Medical Necessity”) shall mean services or supplies provided by a hospital, physician, and/or other health care provider, licensed by the appropriate government agency, or as otherwise approved as required, to identify or treat a condition, disease, ailment, sickness or bodily injury and which, in the opinion of HUMANA, are: (I) consistent with the symptoms, diagnosis and treatment of the condition, disease, ailment, sickness or bodily injury; (II) appropriate with regard to standards of accepted medical practice; (III) not primarily for the convenience of the patient nor the qualified hospital, physician or other health care provider; (IV) the most appropriate and cost-effective supply, setting, or level of service which can safely be provided to the patient; and (V) substantiated by the records and documentation maintained by the provider of services. When applied to an inpatient, it further means that the patient’s symptoms or condition requires that the services or the supplies cannot be safely provided to the patient as an outpatient. Any disputes regarding Medical Necessity shall be handled in accordance with paragraph 11.2 of this Agreement. 12.3 HUMANA, may authorize payment for Medically Necessary Covered Services based on HUMANA’s discretion an...
