Statement of Work to Be Performed. The District is soliciting Bids for Low Voltage Labor Services. The Agreement term for these Services shall be for February 1, 2019 through January 30, 2021, with three (3) one (1) year option.
Statement of Work to Be Performed. The District is soliciting Bids for banner and sign printing, installation, removal and cleaning services. The Agreement term for these services shall be for May 1, 2019, through April 31, 2021, with three (3) one (1) year options.
Statement of Work to Be Performed. The District is soliciting Bids for Videography and video services. The Agreement term for these Services shall be for May 1, 2017, through April 31, 2019, with three (3) one (1) year option.
Statement of Work to Be Performed. The District is soliciting Bids for Site Survey Services. The Agreement term for these Services shall be for December 15, 2018, through February 28, 2019.
Statement of Work to Be Performed. The District is soliciting Bids for Portable Light Tower Rental. The Agreement term for these Services shall be for July 1, 2017 through August 31, 2018 with three (3) one (1) year option.
Statement of Work to Be Performed. A. During the term of this Agreement, Contractor shall provide the Services, implement the tasks, and provide the Deliverables identified herein and in Appendix A, Statement of Work (“SOW”).
B. All work, tasks, and Deliverables are subject to City approval in accordance with the SOW. Failure to receive approval may result in the withholding of compensation for such Deliverable(s) pursuant to Section 6, Compensation and Method of Payment, of this Agreement.
C. Notwithstanding any other provision of this Agreement, the Contractor shall perform such other work and deliver such other items within the scope of Services as are necessary to ensure that the deliverables provided under this Agreement meet the requirements set forth in this Agreement, and all Appendices.
D. In the event that City requires Services in addition to those specified in this Agreement, Contractor agrees to provide such services in accordance with Section 15, Amendments, of this Agreement. Prior to performance of additional work, this Agreement will be amended to include the additional work and payment.
E. Contractor’s performance of the work under this Agreement must not interfere unnecessarily with the operation of LAFD or any other City department. If City, as a result of its own operations, delays, disrupts, or otherwise interferes with and materially affects Contractor’s performance hereunder, adjustments will be determined by mutual agreement of the parties and may be accomplished in accordance with Section 15, Amendments, of this Agreement. Contractor shall notify City immediately if delays, regardless of the cause, begin to put the implementation schedule in jeopardy.
Statement of Work to Be Performed. The District is soliciting Bids for Grounds Sound Audio Services. The Agreement term for these Services shall be for July 1, 2018 through June 30, 2019 with four (4) one (1) year option.
Statement of Work to Be Performed. Contractor will provide a full range of routine hazardous waste management services to the LAFD as described in Exhibit A, Statement of Work (“SOW”). Services will include, but are not limited to: sorting, recycling, treating, categorizing, performing sampling and analysis as needed, packaging, labeling, manifesting, transporting, and disposing of a wide variety of collected hazardous waste from the Department’s various facilities. All work, tasks, and deliverables are subject to Department approval in accordance with the SOW. Failure to receive approval may result in the withholding of compensation for such deliverables(s) pursuant to Section 5.0 – Compensation and Method of Payment of this Agreement. Notwithstanding any other provision of this Agreement, the Contractor will perform such other work and deliver such other items within the SOW as are necessary to ensure that the deliverables provided under this Agreement meet the requirements set forth in this Agreement and all Attachments. In the event that City requires services in addition to those specified in this Agreement, Contractor agrees to provide such services in accordance with Section 7.0 – Amendments and Change Requests of this Agreement. Statement of Work to Be Performed. Contractor agrees to provide, in accordance with the highest professional standards of care, the following services (the "Services"):
A. Meet periodically with nurses and I or Health Services Coordinator to review health concerns and related problems the school district is encountering.
B. Provides clinical consultation on communicable disease outbreaks and emergency medical management.
C. Acts as liaison to community health and human services agencies and facilities in order to assist in interpreting school regulations and establishing school / community health collaboration. (i.e. MEMS, ACH, UAMS, DHS, AAP, ADH, etc.)
D. Recommends equipment and supplies needed in the health service program.
E. Assists in the establishment of standards and quality assurance programs. Annually review the Health Services Operations Manual for accidents and illnesses and makessuggestionsfor improvingstandards of care.
F. Writes prescriptions for stock emergency medication to be used with students and staff and others as needed.
G. Designates a physician as a substitute to provide these services when unavailable.
Statement of Work to Be Performed. As described in the Surface Casing Estimator Site/Web Database FY2023, attached to this Contract as Attachment A and incorporated into this Contract for all purposes.
