Submission of Sample Notification to Attorney General. If notification to more than 500 6 individuals is required pursuant to California Civil Code section 1798.29, and regardless of whether 7 CONTRACTOR is considered only a custodian and/or non-owner of the COUNTY PCI, 8 CONTRACTOR shall, at its sole expense, and at the sole election of COUNTY, either: 9 1. electronically submit a single sample copy of the security breach notification, excluding 10 any personally identifiable information, to the Attorney General pursuant to the format, content, and 11 timeliness provisions of Section 1798.29, subdivision (e). CONTRACTOR shall inform 12 ADMINISTRATOR Privacy Officer of the time, manner, and content of any such submissions, prior to 13 the transmission of such submissions to the Attorney General; or 14 2. cooperate with and assist COUNTY in its submission of a sample copy of the notification 15 to the Attorney General.

Submission of Sample Notification to Attorney General. If the cause of the breach is attributable to the County Department/Agency or an agent, subcontractor, or vendor of the County Department/Agency and if notification to more than 500 individuals is required pursuant to California Civil Code section 1798.29, regardless of whether County Department/Agency is considered only a custodian and/or non-owner of the Medi-Cal PII, County Department/Agency shall, at its sole expense and at the sole election of DHCS, either: 1. Electronically submit a single sample copy of the security breach notification, excluding any personally identifiable information, to the Attorney General pursuant to the format, content, and timeliness provisions of Section 1798.29, subdivision (e). County Department/Agency shall inform the DHCS Privacy Officer of the time, manner, and content of any such submissions prior to the transmission of such submissions to the Attorney General; or 2. Cooperate with and assist DHCS in its submission of a sample copy of the notification to the Attorney General.

Submission of Sample Notification to Attorney General. If notification to more than 500 individuals is required pursuant to California Civil Code section 1798.29, and regardless of whether Recipient is considered only a custodian and/or non-owner of the DATA, Recipient shall, at its sole expense, and at the sole election of CDPH, either: 1. electronically submit a single sample copy of the security breach notification, excluding any personally identifiable information, to the Attorney General pursuant to the format, content and timeliness provisions of Section 1798.29, subdivision (e). Recipient shall inform the CDPH Privacy Officer of the time, manner and content of any such submissions, prior to the transmission of such submissions to the Attorney General; or 2. cooperate with and assist CDPH in its submission of a sample copy of the notification to the Attorney General.

Submission of Sample Notification to Attorney General. If notification to more than 500 individuals is required pursuant to California Civil Code section 1798.29, and regardless of whether Blue Shield is considered only a custodian and/or non-owner of the CDPH PCI, Blue Shield shall, at its sole expense, and at the sole election of the Agency, either: 1. electronically submit a single sample copy of the security breach notification, excluding any Personal Information, to the Attorney General pursuant to the format. content and timeliness provisions of Section 1798.29, subdivision (e). Blue Shield shall inform the CDPH Privacy Officer of the time, manner and content of any such submissions, prior to the transmission of such submissions to the Attorney General; or 2. cooperate with and assist CDPH in its submission of a sample copy of the notification to the Attorney General.

Submission of Sample Notification to Attorney General. If notification to more than 500 individuals is required pursuant to California Civil Code section 1798.29 or 1798.82, and regardless of whether Contractor is considered only a custodian and/or non-owner of the DSH PCI, Contractor shall, at its sole expense, and at the sole election of DSH, either: 1. electronically submit a single sample copy of the security breach notification, excluding any personally identifiable information, to the Attorney General pursuant to the format. content and timeliness provisions of Section 1798.29, subdivision (e), or 1798.82, subdivision

Submission of Sample Notification to Attorney General. If notification to more than 500 individuals is required pursuant to California Civil Code section 1798.29, regardless of whether the contractor is considered only a custodian and/or non-owner of the Agency on Aging PSCI, the contractor shall, at its sole expense and at the sole election of the Agency on Aging, either: i. Electronically submit a single sample copy of the security breach notification, excluding any personally identifiable information, to the Attorney General pursuant to the format, content, and timeliness provisions of Section 1798.29, subdivision (e). The contractor shall inform the Agency on Aging Privacy Officer: ▇▇▇▇▇▇▇▇ ▇▇▇▇▇▇▇, of the time, manner, and content of any such submissions prior to the transmission of such submissions to the Attorney General; or ii. Cooperate with and assist the Agency on Aging in is submission of a sample copy of the notification to the Attorney General.
