SUBRECIPIENT’S Program Enrollments shall be as follows. 1. Each young adult is enrolled in the program for an expected term linked to the Individual Service Plan (ISP). 2. All young adults must receive Post-Exit Follow-Up Services for a minimum of twelve (12) months after exit. 3. SUBRECIPIENT shall meet the monthly enrollment goals as identified in Attachment D of the CONTRACT.
