Survey Questionnaire Clause Samples

The Survey Questionnaire clause defines the requirements and procedures for creating, distributing, and collecting responses to a survey as part of an agreement or project. Typically, this clause outlines who is responsible for drafting the questionnaire, the timeline for its completion, and the method of distribution to participants. It may also specify standards for the content and format of the questions to ensure consistency and relevance. The core function of this clause is to ensure that all parties have a clear understanding of how survey data will be gathered, thereby promoting transparency and reliability in the information collection process.
Survey Questionnaire. The survey questionnaire was designed after reviewing the instruments used in similar kind of household studies in Kenya and prototype survey instruments available from the user guide of the World Bank Group (Preparing Surveys for Urban Upgrading Interventions – Prototype Survey Instrument and User Guide, March 2008). The questionnaire used contains six modules, namely: demographics and household composition; security of housing, land and tenure; settlement profile; economic and employment profile; infrastructure services; and health
Survey Questionnaire. Figure 6 shows two sample questions from our survey questionnaire that encompass the diverse question types we describe in this section. Each of our questions falls under primarily one of the following three types of questions.
Survey Questionnaire i. Develop and distribute questionnaire(s) to County stakeholders regarding forthcoming project issues, as specified by the County Project Officer. ii. Conduct meetings with County Staff and community to determine structure of and questions for survey. iii. Prepare questions, formatting, printing, distribution and collection of surveys.
Survey Questionnaire. The LQAS questionnaire was developed by adapting the previously successful baseline KPC questionnaire. The questions pertaining to the indicators in the project matrix were chosen and placed in the LQAS questionnaire, and to this additional questions were added to provide indicators in intervention areas that the project may have interest in adding to the project. A questionnaire was developed for each category (denominator) of child needed to be interviewed that contained the questions pertinent to that child’s category. There were a total of 7 questionnaires developed for the following categories:
Survey Questionnaire. The questionnaire was adapted from the standard Malaria Indicator Survey. However, the survey differed from the standard Malaria Indicator Survey in that no blood tests were conducted and some of the indicators were specific to the research study. The questionnaire contained 20 questions classified into five categories: household ownership of LLINs, household LLIN coverage, net usage, net hang-up methods and net condition, and the head of the household’s knowledge of correct use of the nets. The questionnaires were translated into French and administered in Lingala by the interviewer. The interviews were conducted by the principal investigator, who spoke Lingala. The interviewer explained the purpose of the survey to each head of the household before administering the survey or conducting any observations. Signed consent was obtained from each head of the household.
