Deductions from sick, personal, professional, bereavement or vacation will be made in the business office. When completing your leave form, please make recommendations for your choice of substitute to be hired. If you do not take leave for the time requested, please submit a note or a photocopy of the leave slip with the corrections, to the Superintendent or business office. Eligibility: FMLA requires covered employers to provide up to 12 weeks of unpaid, job-protected leave to eligible employees. Employees must request leave and be determined to be eligible to receive FMLA leave. Please contact the Superintendent for additional information and proper forms.
