Termination of Coverage. a. An employee on leave whose payment of premium is in arrears will be dropped from coverage and may not re-enroll without successfully completing the Insurance Carrier Contract requirements for re-enrollment during the annual open enrollment. b. The death of the employee on leave will cause termination of all coverages. c. A voluntary withdrawal from participation in this plan shall terminate coverage for employees on leave.
