This Amendment to Amended Agreement for Services at an Onsite Center (the “Amendment”) is made and entered into as of the day of June, 2023 (the "Amendment Effective Date"), by and between Concentra Health Services, Inc., a Nevada corporation, for the benefit of and on behalf of its subsidiaries, affiliates, and managed professional associations and corporations (“Concentra") and the Town of Palm Beach, a Florida municipality (“Client”).
